Hallucinated a loan amount on a real customer call

We caught it in the call review the next morning. The agent told a customer they were approved for a number that was not in the prompt or the API response. Bland support was responsive when we escalated but the root cause never got a clean answer. Pulled it from production same week. The promise is real, the production polish on long context calls is not there for us yet.

Looped on the transfer to a human

Verified

Customer asked to speak to a person. Agent acknowledged, said it would transfer, then started the call over. Repeat. We watched it happen on three live calls before pulling the plug. Bland support said it was a configuration issue but we couldnt find any prompt or pathway change that would explain it. Cant trust a vendor where this kind of regression sneaks in.

Quoted wrong appointment slots on three calls in a row

Verified

Patients were getting confirmed for slots that didnt exist on our doctors calendar. We checked the prompts, checked the API responses going into the agent, the source of truth said one thing and the agent said another. Bland support eventually got back to us but by then we'd already lost the trust of two clinic partners. Pulled it from production and went back to a human scheduler. Maybe its better at simpler use cases.

Verified

Initial deployment was rougher then expected — a lot of edge-case prompt tuning. After three months it does the appointment-reminder use case well. Wouldnt trust it on nuanced calls without close monitoring.

Hallucinated an account balance in a sandbox demo

Verified

We were doing a sandbox demo for our compliance team to walk through controls and the agent confidently told the test caller their balance was a number that doesnt match any API response we logged. The team caught it because they knew the test data but in prod thats a real risk. We added more guardrails and constrained the prompt, hallucination rate dropped but didnt zero out. Live with it for now.
